Succeeding in Clinically Integrated Networks

Clinically integrated networks can help physicians work together to respond to value-based care. This model includes accountability for patient outcomes, which involves a level of risk assumption. This one-hour, 1-credit CME will help participants better understand the benefits and accountability involved in Clinically Integrated Network participation. It will provide a comprehensive review of how risk can be measured, transferred and monetized to support value-based care. Key capabilities such as population health management and team-based care management will also be examined.
1-hour CME awarded to physicians; 1-hour CEU awarded to practice administrators
Following the CME and a short recess, there will be a brief presentation on the Pennsylvania Medical Society’s new Clinically Integrated Network for independent practices. This new and true physician-led CIN is an opportunity for fiercely independent physicians in the region to work together – and with the proper structure, population health platform and care management services, that will allow the doctors to chart their own future and help develop meaningful value-based contracts.
